If you would like to continue your care with them, please contact the clinic and complete a Consent to Transfer Medical Records. This will need to be emailed to admin@healthicare.org.au by Glenmore Park Mediclinic administration staff. 
  • If you plan to see a different GP, we also require your consent to transfer your medical records to your new provider.  Please contact the clinic and complete a Consent to Transfer Medical Records. This will need to be emailed to admin@healthicare.org.au by the clinic administration staff.
