FAQs
At Healthicare, we are committed to providing comprehensive care that addresses the social determinants of poor health outcomes for disadvantaged and vulnerable communities. We have partnered with the Mt Druitt Medical Practitioners Association (MDMPA) to create a Neighbourhood Health Hub that serves as an extension to neighbouring general practices and other providers. Our goal is to provide access to wraparound services that support those with complex care needs.
Supported by WentWest, in partnership with the Mt Druitt Medical Practitioners Association (MDMPA), Healthicare aims to work collaboratively with health care professionals to deliver targeted and integrated primary care services, through a transformative collaborative neighbourhood-based model of care and improve the health and social outcomes of the most vulnerable families.
