Healthicare will work collaboratively with health care professionals, supporting the delivery of targeted and integrated primary care services, through a transformative collaborative neighbourhood- based model of care, and improve the health and social outcomes of the most vulnerable families. This includes extremely socio-economically disadvantaged patients with multiple co-morbidities.

To minimise barriers to accessing health care services, Healthicare aims to address the long waitlists in our region and will provide no out-of-pocket costs for patients.

The service model will:

  • Support the delivery of high quality evidence-based and patient-centred primary health care aligned to the Patient Centred Medical Home model and enhanced specialised primary care models.
  • Provide collaborative referral pathways to and from the Healthicare hub and other service providers to increase access and availability of services.
  • Have a dedicated multidisciplinary clinical and non-clinical team supplemented by a range of visiting and neighbourhood-based service providers.
  • Provide a spectrum of holistic primary care services and referrals to social services required to support vulnerable families.

Team-based care team

Healthicare’s team-based care team is a critical element in transforming the way care is delivered. A care team is a small group of clinical and non-clinical staff who, together with a provider, are responsible for the health and wellbeing of a population of patients.

What is Healthicare?

At Healthicare, we are committed to providing comprehensive care that addresses the social determinants of poor health outcomes for disadvantaged and vulnerable communities. We have partnered with the Mt Druitt Medical Practitioners Association (MDMPA) to create a Neighbourhood Health Hub that serves as an extension to neighbouring general practices and other providers. Our goal is to provide access to wraparound services that support those with complex care needs.

Why is Healthicare being established in Western Sydney?

Western Sydney is an incredibly diverse region, home to 1,080,817 people with 57% of the region using a language other than English at home (Australian Bureau of Statistics (2021)).

What services are available?

Healthicare aims to cater for everyone who requires holistic health services, utilising GPs with special interest in child and family, complex chronic conditions and mental health as a start, although we expect to be expanding our services and capacity as per needs.

Our aim is to wrap health care services into one unified Neighbourhood Health Hub, supporting surrounding general practice with access and services for those with complex care needs.

We offer general GP services, in addition to patient-activated measures, helping assist patients navigate problems you may have with your health from a new perspective.

Who can access Healthicare?

Healthicare is available to anyone who requires access to health care services we offer. We support neighbouring GPs who may wish to refer their patients and are also available to community members who do not have a regular GP.

We are especially looking forward to caring for families with young children, complex chronic conditions and mental health concerns.

Do patients need a referral?

No referral is required to book an appointment.

Are services bulk-billed?

Yes, Healthicare is a bulk-billing general practice and all eligible services are bulk-billed for patients with a valid Medicare card.

Healthicare is a locally-led initiative, established in partnership with the Mt Druitt Medical Practitioners Association and was supported by WentWest. Healthicare will work with many partners spanning the broader health and social sector, including allied health, community health, hospitals, community organisations, local schools and more.
